Athletic Participation Packet

All students trying out for a school sport are required to turn in an athletic participation packet prior to the start of each school year.

Student-athletes will not be allowed to participate in any tryouts, practices, scrimmages, or games until their athletic participation packet is completed and on file with the athletic department by the specified dates below.

                       Fall: July 30, 2021

                       Winter: October 25, 2021

                       Spring: January 24, 2022

The athletic participation packet consists of the forms below:

  1. Pre-Participation Physical Exam Form (please download and complete with your physician)
  2. Athletic Emergency Form
  3. Consent and Agreement Form
  4. Concussion and Head Injury Management Form
  5. Sudden Cardiac Awareness Information Form
  6. Heat Illness Information Form
  7. Opiod Information Form

Forms 2-7 may be signed and submitted electronically logging into SportsWare – forms are housed under “Attachments” when you login. The pre-participation physical exam form may also be scanned and submitted through SportsWare under the Attachments tab, or you may forward a copy to the school at the attention of Athletic Trainer. If you mail a hard copy of the physical, it is highly recommended that you retain a copy for your records.

Please note you must register and complete forms for each student-athlete separately.

If the student is selected for a team roster, an online registration and fee is required for each sport season. This online registration requirement will open in conjunction with each sport season mandatory parent night and must be completed by the due date announced at the parent meeting.
