Athletic Participation Packet

All students trying out for a school sport are required to complete the Magnus Health packet prior to the start of each school year.

Student-athletes will not be allowed to participate in any tryouts, practices, scrimmages, or games until their Magnus Health packet is completed. 

You must indicate in your Magnus Health form that the student athlete intends to play sports at Presentation.  This will add the additional mandatory Athletic Department forms to your Magnus account. 

The Athletic Participation Packet has been moved to Magnus Health and consists of the forms below:

1. Annual Physical Examination Form
2. Athletic Emergency Form
3. Consent and Agreement Form
4. Concussion and Head Injury Management Form
5. Sudden Cardiac Awareness Information Form
6. Heat Illness Information Form
7. Opioid Information Form

Please note you must register and complete forms for each student-athlete separately.

If the student is selected for a team roster, an online registration and fee is required for each sport season. This online registration requirement will open in conjunction with each sport season mandatory parent meeting and must be completed by the due date announced at the meeting.
